La tuberculosis es una enfermedad infecto-contagiosa producida por Mycobacterium tuberculosis, su presencia se relacionó en el pasado con alta mortalidad, sin embargo los adelantos en la terapéutica y la mejoría en las condiciones de vida hicieron que la enfermedad sea susceptible de control y que la muerte se pueda evitar. No obstante, las poblaciones en extrema pobreza como los habitantes de calle continúan siendo afectados por esta enfermedad en sus formas agresivas, además sus condiciones de vida hacen difícil el cumplimiento y efectividad del tratamiento. En Bogotá la población Habitante de calle se estimó en 8.385, según censo 2007, la Alcaldía adelanta desde 2009 un programa específico de control de tuberculosis dirigido a esta población. Por ello el presente estudio tuvo como objetivo: Evaluar las acciones del Programa de Prevención y Control de Tuberculosis de la Secretaria Distrital de salud de Bogotá enmarcada en la Estrategia "Alto a la Tuberculosis" llevada a cabo en el Hogar uno día noche en la atención del Habitante de calle durante los años 2009 y 2010. Se realizó un: estudio de caso y evaluación con diseño descriptivo, retrospectivo de las acciones desarrolladas por el Programa de Prevención y Control de Tuberculosis según la Organización Mundial de la Salud bajo la Estrategia "Alto a la TB", incluyo: revisión documental, entrevistas semiestructuradas y análisis de indicadores. Los resultados mostraron que: el Programa estableció una relación coordinada entre el sector de integración social y el sector salud y demostró su capacidad de operar en forma continua, atendiendo el 32% del total de casos de tuberculosis ocurridos en la población Habitante de calle. La acción del Programa permitió obtener un éxito de tratamiento de 39%, afectado principalmente por el abandono de la terapéutica. En conclusión; el reconocimiento del derecho a la salud de los habitantes de calle determino que acciones intersectoriales favorecieran el acceso a los servicios de salud y facilitó acciones de educación, diagnóstico y seguimiento a los enfermos con TB.
Abstrait
Tuberculosis is a contagious infectious disease caused by Mycobacterium tuberculosis, its presence was associated in the past with high mortality , however therapeutic advances and improved living conditions made it susceptible to disease control and the death can be avoided . However , populations in extreme poverty as street dwellers continue to be affected by this disease in their aggressive ways besides their living conditions make compliance and effectiveness of treatment difficult. Living In Bogota the street population was estimated at 8,385 , according to 2007 census , the City advances since 2009 a specific tuberculosis control program aimed at this population. Therefore this study aimed to : Evaluate the actions of the Programme for Prevention and Control of Tuberculosis of the District Department of Health in Bogotá Framed " Stop TB " strategy carried out in the evening at home one day care Dweller street during 2009 and 2010. Case study and evaluation with descriptive , retrospective design of the actions developed by the Programme for Prevention and Control of Tuberculosis according to the World Health Organization under the " Stop TB " strategy , I include : -a was performed document review , interviews semistructured and analysis of indicators . The results showed that : the Program established a coordinated relationship between the area of social integration and health sector and demonstrated its ability to operate continuously , taking 32% of all cases of tuberculosis occurred in the Living street population . The action program yielded successful treatment of 39 % , mainly affected by the discontinuation of therapy. In conclusion , the recognition of the right to health of the inhabitants of street determined that intersectoral action favored access to health services and provided the education, diagnosis and monitoring of patients with TB.
