Características operativas de la prueba ALIFAX / Meropenem para la detección de bacilos Gram negativos resistentes a meropenem a partir de hemocultivos

El presente trabajo tiene como objetivo determinar las características operativas y la concordancia de la prueba ALIFAX/MEROPENEM para la detección de bacilos Gram negativos resistentes a meropenem a partir de hemocultivos, con el fin de determinar el tiempo de oportunidad en la detección de bacilos Gram negativos resistentes a meropenem por el sistema ALIFAX/meropenem comparado con el tiempo de oportunidad de la microdilución en caldo.
En este estudio se evaluaron 100 hemocultivos positivos para bacilos Gram negativos, monomicrobianos verificados con coloración de Gram, se realizó identificación bioquímica y antibiograma para meropenem por medio del equipo automatizado VITEK®2. El resultado se informó en Concentración Mínima Inhibitoria (MIC) según CLSI para cada microorganismo; al mismo tiempo partiendo del hemocultivo se realizó la prueba ALIFAX/meropenem, metodología que tiene como principio la nefelometría laser. Los resultados obtenidos por el equipo ALIFAX/meropenem se reportaron en Porcentaje de Capacidad Inhibitoria (PIC) donde de 0-35% se indicaba la presencia de un microorganismo sensible a meropenem y de 36-100%se indicaba un microorganismo resistente a meropenem.
La prueba ALIFAX/meropenem presento una sensibilidad de 62,50%, una especificidad de 97,60%, un valor predictivo positivo (VPP) de 83,30% y un valor predictivo negativo (VPN) de 93,20%; además de esto se obtuvo una fuerza de concordancia casi perfecta con un índice Kappa de 0,91. En cuanto al tiempo de oportunidad de detección la técnica ALIFAX/meropenem reduce el tiempo de procesamiento en 19,94 horas en comparación al estándar de oro.
Aunque la prueba reporta un índice de concordancia casi perfecto, una especificidad similar a la del estándar de oro y reduce los tiempos de gran manera, posee una sensibilidad por debajo del 90%, obteniendo una gran cantidad de falsos sensibles, que compromete el tratamiento y las medidas de contención de la diseminación de resistencia a nivel intrahospitalario, por lo tanto, es importante evaluar las posibles causas de falsos sensibles y las limitaciones encontradas durante el estudio.
Astratto
The objective of this work is to determine the operative characteristics and concordance of the ALIFAX / MEROPENEM test for the detection of Gram-negative bacillus resistant to meropenem from blood cultures, in order to determine the time of opportunity in the detection of Gram-negative bacillus resistant to meropenem by the ALIFAX / meropenem system compared to the opportunity time of broth microdilution.
In this study, 100 positive blood cultures were evaluated for Gram negative bacillus, monomicrobials verified with Gram stain, biochemical identification and antibiogram for meropenem were performed by means of automated VITEK®2 equipment. The result was reported in Minimum Inhibitory Concentration (MIC) according to CLSI for each microorganism; at the same time, starting from the blood culture, the ALIFAX / meropenem test was performed, a methodology that is based on laser nephelometry. The results obtained by the ALIFAX / meropenem team were reported as Percentage of Inhibitory Capacity (PIC) where 0-35% indicated the presence of a microorganism sensitive to meropenem and 36-100% indicated a microorganism resistant to meropenem.
The ALIFAX / meropenem test showed a sensitivity of 62.50%, a specificity of 97.60%, a positive predictive value (PPV) of 83.30% and a negative predictive value (NPV) of 93.20%; in addition to this, an almost perfect match strength was obtained with a Kappa index of 0.91. Regarding the detection opportunity time, the ALIFAX / meropenem technique reduces the processing time by 19.94 hours compared to the gold standard.
Although the test reports an almost perfect concordance index, a specificity similar to that of the gold standard and greatly reduces the time, it has a sensitivity below 90%, obtaining a large number of false sensitive, which compromises the treatment and The measures to contain the spread of resistance at the hospital level, therefore, it is important to evaluate the possible causes of false sensitive and the limitations found during the study.
