Clinical factors associated with high glycemic variability defined by coefficient of variation in patients with diabetes mellitusResumen
Antecedentes: la alta variabilidad glucémica (HGV) se ha convertido en un predictor más fuerte de hipoglucemia. Sin embargo, los factores clínicos asociados con el HGV aún son desconocidos.
Objetivo: determinar las variables clínicas que se asociaron con un coeficiente de variación (CV) superior al 36% evaluado mediante monitoreo continuo de glucosa (MCG) en un grupo de pacientes con diabetes mellitus.
Métodos: una cohorte de pacientes con diabetes tipo 1 (T1D) y tipo 2 (T2D). Se evaluaron variables demográficas, A1c, tasa de filtración glomerular (TFG) y actividad física. Se realizó un análisis bivariado, para el cual se realizó un modelo de regresión logística dicotómica entre la variable de resultado (CV> 36%) y cada una de las variables independientes. Finalmente, se construyó un modelo multivariado que incluía las variables del análisis bivariado y se seleccionó el mejor modelo a través de pruebas de bondad de ajuste como el AIC y el BIC.
Resultados: se analizaron los datos de MCG de 350 pacientes. CV> 36% estuvo presente en 89 pacientes (24,6%). En el análisis bivariado, se incluyeron variables demográficas y clínicas, como el tiempo desde el diagnóstico, la actividad física, el tratamiento con insulina, el historial de hipoglucemia, A1c y TFG. En el análisis multivariado, TFG <45 ml / min (OR 2.29, IC 95% 1.19-4.38, p0.012), A1c> 9% (OR 2.78, IC 95% 1.25-6.20, p0.012) e hipoglucemia histórica ( O 1.78, IC95% 1.01–3.15, p0.000) se asociaron con HGV.
Conclusión: La determinación de variables clínicas tales como bajo FG, control glucémico deficiente y antecedentes de hipoglucemia que están asociados con el VGH, por lo tanto, un mayor riesgo de hipoglucemia. Esta población debe considerarse para los tratamientos que pueden reducir la variabilidad glucémica y la MCG.
Abstract
Background: High glycemic Variability (HGV) has become a stronger predictor of hypoglycemia. However, clinical factors associate with HGV still are unknown.
Objective: To determine clinical variables that were associated with a coefficient of variation (CV) above 36% evaluated by continuous glucose monitoring (CGM) in a group of patients with diabetes mellitus.
Methods: A cohort of patients with type 1 (T1D) and type 2 (T2D) diabetes. Demographic variables, A1c, glomerular filtration rate (GFR) and physical activity were assessed. A bivariate analysis was performed, for which a dichotomous logistic regression model was performed between the outcome variable (CV> 36%) and each of the independent variables. Finally, a multivariate model was constructed including the variables of the bivariate analysis and the best model was selected through goodness of fit tests such as the AIC and the BIC.
Results: CGM data from 350 patients were analyzed. CV > 36% was present in 89 patients (24.6%). In the bivariate analysis, demographic and clinical variables were included such as time since diagnosis, physical activity, insulin treatment, hypoglycemia history, A1c and GFR. In the multivariate analysis, GFR <45 ml/min (OR 2.29, 95% CI 1.19-4.38, p0.012), A1c > 9% (OR 2.78, 95% CI 1.25-6.20, p0.012) and history hypoglycemia (OR 1.78, IC95% 1.01–3.15, p0.000) were associated with HGV.
Conclusion: The determination of clinical variables such as low GFR, poor glycemic control and history of hypoglycemia that are associated with HGV therefore higher risk of hypoglycemia. This population should be considered for treatments who can reduce glycemic variability and CGM.
