Comparison of peri-implant radiographic bone height in patients with platform switching dental implants and platform matching dental implants: 12-month retrospective studyResumo
Antecedentes: El uso de implantes dentales con reducción de plataforma se ha propuesto como una alternativa para disminuir la pérdida ósea marginal alrededor de implantes dentales debido a que se aleja el microgap protésico de la cresta ósea en sentido horizontal. Algunos estudios han concluido que esta característica en el implante puede mantener o incluso aumentar el nivel óseo peri-implantar a nivel crestal a mediano y largo plazo en comparación con implantes sin reducción de plataforma. Sin embargo, la literatura reciente no es concluyente en cuanto al beneficio con respecto a la preservación de altura ósea peri- implantar en los implantes con reducción de plataforma.
Objetivo: Comparar la altura ósea radiográfica peri-implantar en pacientes con implantes sin reducción de plataforma e implantes con plataforma reducida.
Materiales y métodos: Se realizó un estudio retrospectivo en el que se analizaron las radiografías periapicales existentes incluidas en las historias clínicas digitales de pacientes a los que se le colocaron implantes dentales en la Facultad de Odontología de la Pontificia Universidad Javeriana, entre enero de 2018 y enero de 2021. Se analizaron dos radiografías para cada implante, la inicial en el momento de la colocación de la rehabilitación definitiva y la final, con un seguimiento de 12 meses. Se evaluaron un total de 50 pacientes, 35 hombres y 15 mujeres, de los cuales a 37 pacientes se le colocaron implantes sin reducción de plataforma (grupo 1: 65 implantes) y a 13 pacientes se les colocaron implantes con plataforma reducida (grupo 2: 18 implantes). Utilizando el software CS Imaging Software Carestream, para disminuir los errores de medición, se estandarizaron las radiografías con la herramienta de proporcionalidad donde se incluyen las dimensiones de los implantes registradas en las historias clínicas y posteriormente se midió la distancia desde la plataforma del implante hasta la cresta ósea, tanto mesial como distal de los implantes, tomando como punto de referencia la plataforma del implante. Estas distancias representan los cambios en los niveles óseos peri-implantarios. De esta manera, las distancias en posición apical a la plataforma del implante se registraron con valores negativos y las distancias coronales a la
plataforma se registraron con valores positivos. De estos valores obtenidos se sacó un promedio el cual permitió comparar la pérdida ósea entre ambos grupos.
Resultados: Se encontró significativamente (p<0,05) mayor pérdida ósea peri-implantar alrededor de implantes sin reducción de plataforma, con los siguientes valores: mesial (-1,64 ± 1,32 mm) y distal (-1,67 ± 1,17 mm), así como el promedio (-1,67 ± 1,18 mm). Mientras que para el grupo de implantes con reducción de plataforma no se encontró pérdida ósea peri-implantar, mesial (0,23 ± 1,36 mm) y distal (0,36 ± 1,46 mm) y el promedio de ambas (0,30 ± 1,38 mm).
Conclusiones: Existe beneficio en el uso de implantes con plataforma reducida para preservar el nivel óseo peri-implantar en un periodo de 12 meses posterior a la carga del implante.

Abstract
Background: The use of platform switching implants has been proposed as an alternative to reduce marginal bone loss around dental implants because the prosthetic microgap moves away from the bone crest horizontally. Some studies have concluded that this feature in the implant can maintain or even increase the peri-implant bone level at the crestal level in the medium and long term compared to implants without platform switching. However, the recent literature is not conclusive regarding the benefit regarding the preservation of peri- implant bone height in implants with platform reduction.
Objective: To compare the peri-implant radiographic bone height in patients with implants without platform reduction and implants with reduced platform.
Materials and methods: A retrospective study was carried out in which the existing periapical radiographs included in the digital medical records of patients who had dental implants placed at the Pontificia Universidad Javeriana were analyzed between January 2018 and January 2021. Two radiographs were analyzed for each implant, the initial one at the time of placement of the definitive restoration and the final one, with a 12-month follow-up. A total of 50 patients, 35 men and 15 women, were evaluated, of which 37 patients received implants without platform reduction (group 1: 65 implants) and 13 patients received implants with reduced platform (group 2: 18 implants). Using the CS Imaging Carestream Software to reduce measurements errors, the radiographs were standardized with the proportionality tool where the dimensions of the implants recorded in the medical records are included and subsequently the distance from the implant platform to the implant bone crest was measured, both mesial and distal of the implants, taking the implant platform as a reference point. These distances represent the changes in the peri-implant bone levels. In this way, the distances apical to the implant platform were recorded as negative values and the coronal distances to the platform were recorded as positive values. From these values obtained which allowed comparing bone loss between both groups.
Results: Significantly (p<0,05) greater peri-implant bone loss was found around implants without platform reduction, with the following values: mesial (-1,64 ± 1,32 mm) and distal (-1,67 ± 1,17 mm), as well as the average (-1,67 ± 1,18 mm). While for the group of implants with platform reduction no peri-implant bone loss was found, mesial (0,23 ± 1,36 mm) and distal (0,36 ± 1,46 mm) and the average of both (0,30 ± 1,38 mm).
Conclusion: There is benefit in the use of implants with a reduced platform to preserve the peri-implant bone level in a period of 12-months after implant loading.
