Determinación de la sensibilidad antifúngica en aislamientos clínicos de Candida auris asociados a procesos invasivos y de colonización recuperados en Cartagena, Colombia

C. auris es un hongo emergente multiresistente a antifúngicos utilizados a nivel clínico para el tratamiento de enfermedades fúngicas de importancia clínica, la determinación de la susceptibilidad antifúngica en esta levadura constituye una herramienta principal para el tratamiento de las infecciones. La prueba de susceptibilidad antifúngica desempeña un papel útil en el manejo de las infecciones por Candida spp y en la vigilancia de la resistencia por estos microrganismos. La técnica micro dilución en caldo se constituye como el estándar de oro para la determinación de la susceptibilidad antifúngica proporcionando resultados confiables y con un alto porcentaje de concordancia entre laboratorios.
El objetivo de este trabajo es determinar la suceptibiliadad antifúngica de aislamientos clínicos de C. auris provenientes de procesos de colonización e invasivos mediante la técnica microdilución en caldo siguiendo los protocolos del CLSI (M27A3) y entender el comportamiento de esta levadura con el hospedero. Para implementar la técnica de microdilución en caldo primero se determinó la CIM de 10 cepas de C. auris cuyo perfil de susceptibilidad ya se conocía el cual había sido determinado por el CDC (center for disease control and prevention) por sus siglas en inglés, laboratorio de referencia mundial y cuyos resultados debían ser reproducibles con un margen de ± 2 a las concentraciones dadas en el CDC.
En total se evaluaron 50 cepas provenientes de Cartagena, Colombia una de las ciudades con mayor número de casos de infecciones por este patógeno. De las 50 cepas de C. auris, 27 provienen de procesos invasivos y 23 de procesos de colonización. Para cada cepa se realizó la susceptibilidad antifúngica por duplicado para confirmar su concentración inhibitoria mínima (CIM).
Con este estudio se espera determinar la sensibilidad antifúngica de C. auris provenientes tanto de procesos de colonización como de procesos invasivos en aislamientos recuperados en Cartagena para conocer el perfil de susceptibilidad antifúngica de la levadura que se encuentran circulando en esta región del país y entender el comportamiento de esta levadura con el hospedero.
Resultados: se implementó la técnica de micro dilución en caldo para la determinación de sensibilidad fúngica en 10 aislamientos clínicos de C. auris con sensibilidad antifúngica conocido cuyos datos tenían que ser reproducibles. De estas cepas de C. auris tuvieron 100% de reproducibilidad de CIM a los cuatro antifúngicos en estudio con los reportados por el CDC utilizando la técnica de microdilución en caldo, validándose la implementación de esta técnica en el laboratorio.
Se determinó el perfil de sensibilidad antifúngica mostrando un 98,5% de sensibilidad a los principales antifúngicos empleados a nivel clínico, El 1% de las cepas de C.auris dio resistente a Voriconazol y el 0.5% de las cepas dio SSD a Fluconazol. No se evidencio diferencias significativas del perfil de sensibilidad antifúngica tanto en aislamientos invasivos y colonizantes, esto sugiere que los aislamientos invasivos no necesariamente pueden generar perfil de resistencias en el hospedero y contribuir a un mal pronóstico clínico.
Conclusión: Se destaca en este estudio la realización de la técnica microdilucion en caldo, técnica gold estándar en el estudio de sensibilidad antifungica. Los resultados obtenidos en la determinación de la sensibilidad antifungica contribuyen al conocimiento de la epidemiología de C. auris en Cartagena y en el país también promueve a seguir trabajando para consolidar estudios en pro de conocer la sensibilidad antifúngica de C. auris que se encuentran circulando en el país.
Se recomienda analizar los datos obtenidos en esta investigación con estudios en los factores de virulencia que ayuden a entender más sobre el comportamiento de este microorganismo emergente con el hospedero.
Abstrait
C. auris is an emerging fungus. The antifungal susceptibility test plays a useful role in the management of Candida spp infections and in the surveillance of resistance by these microorganisms. The technique of micro dilution in time is considered as the gold standard for the determination of antifungal susceptibility of reliable results and with a high percentage of agreement between laboratories.
The objective of this work is to determine the suceptibility. Antifungal of clinical isolates of C. auris from colonization and invasive processes using the microdilution technique in broth following the CLSI protocols (M27A3) and understanding the behavior of this yeast with the host. To implement the microdilution technique, the CIM of 10 C. auris strains whose susceptibility profile was already known which had been given by the CDC (center for the control and prevention of diseases) by its initials was determined. English, world reference laboratory and that the results are reproduced with a margin of ± 2 to them in the CDC.
A total of 50 strains were evaluated from Cartagena, Colombia. Of the 50 strains of C. auris, 27 come from invasive processes and 23 from colonization processes. For each strain the antifungal susceptibility was performed in duplicate to confirm its minimum concentration (MIC).
With this study we can determine the antifungal sensitivity of C. auris from both colonization processes and invasive processes in Cartagena to know the antifungal susceptibility profile of the yeast that is circulating in this region of the country and understand the behavior of this yeast with the host.
Results: the technique of micro dilution in broth was implemented for the determination of fungal sensitivity in 10 clinical isolates of C. auris with known antifungal sensitivity whose data had to be reproducible. Of these strains of C. auris had 100% reproducibility of MIC to the four antifungals under study with those reported by the CDC using the broth microdilution technique, validating the implementation of this technique in the laboratory. The antifungal sensitivity profile was determined, showing a 98.5% sensitivity to the main antifungals used clinically, 1% of the strains of C. auris gave voriconazole resistant and 0.5% of the strains gave SSD to Fluconazole. No significant differences were found in the profile of antifungal sensitivity in both invasive and colonizing isolates, this suggests that invasive isolates may not necessarily generate resistance profiles in the host and contribute to poor clinical prognosis. Conclusion: This study highlights the performance of the microdilution technique in broth, standard gold technique in the study of antifungal sensitivity. The results obtained in the determination of the antifungal sensitivity contribute to the knowledge of the epidemiology of C. auris in Cartagena and in the country it also promotes to continue working to consolidate studies in order to know the antifungal sensitivity of C. auris that are circulating in the country. It is recommended to analyze the data obtained in this investigation with studies on the virulence factors that help to understand more about the behavior of this emerging microorganism with the host.
