Effect of scaling and root planning in hypertensive patients on serum levels of IL 1-β, IL-4, IL-6, IL-8, IL-10, IL-12 P70, IL-17A, VEGF and TNF -α.Resumen
Resumen
Antecedentes: La literatura científica ha corroborado la relación que hay entre la hipertensión y la enfermedad periodontal. La terapia periodontal no quirúrgica, como el raspaje y alisado radicular a campo cerrado, puede tener un efecto sobre marcadores sistémicos de la inflamación.
Objetivo: Evaluar el efecto del raspaje y alisado radicular a campo cerrado sobre los niveles séricos de biomarcadores de inflamación en pacientes hipertensos.
Materiales y métodos: La muestra fue de 19 pacientes hipertensos, con diagnóstico periodontal Periodontitis Estadio II, III y IV, Grado A y B. Se les realizaron exámenes de laboratorio que incluían: glicemia, colesterol (LDL, HDL), triglicéridos y cuadro hemático completo. Se les realizó la toma de tensión arterial antes de iniciar la terapia periodontal y en la cita de reevaluación se tomó el segundo registro de tensión arterial. La cuantificación de las citocinas de sangre periférica se realizó mediante el kit Milliplex Inflammation Human Cytokine.
Resultados: Todos los niveles de citocinas medidas en los pacientes de la muestra disminuyeron del examen inicial a la reevaluación. La IL-1B tuvo un valor inicial de 0.53 pg/ml, la IL-4 de 2.98 pg/ml, IL-6 de 0.31 pg/ml, IL-8 de 9.17 pg/ml, la IL-10 de 1.15 pg/ml, IL-12P70 de 1.82 pg/ml, IL-17A de 3.8 pg/ml, TNFa de 15.84 pg/ml y el VEGF de 152.10 pg/ml. Las medidas luego del tratamiento periodontal fueron: IL-1de 0.30 pg/ml, IL-4 de 1.23 pg/ml, IL-6 de 0.24 pg/ml, IL-8 de 7.79 pg/ml, IL-10 de 0.86 pg/ml, IL-12P70 de 0.94 pg/ml, IL-17A de 3.19 pg/ml, TNFa de 14.29 pg/ml y VEGF de 101.55 pg/ml. Las citocinas que reflejaron una diferencia estadísticamente significativa fueron la IL-1B y VEGF (p=0.04 y p=0.004).
Conclusiones: Los pacientes hipertensos con periodontitis sometidos a tratamiento periodontal no quirúrgico presentaron una disminución en los niveles de citocinas pro inflamatorias evaluadas, aunque las únicas que presentaron una diferencia estadísticamente significativa fue la VEGF y la IL-1B.

Background: The scientific literature has corroborated the relationship between hypertension and periodontal disease. Nonsurgical periodontal therapy, such as scaling and root planning, can have an effect on systemic markers of inflammation.
Objective: To evaluate the effect of scaling and root planning on serum levels of biomarkers of inflammation in hypertensive patients.
Materials and methods: The sample consisted of 19 hypertensive patients, with periodontal diagnosis Periodontitis Stage II, III and IV, Grade A and B. They underwent laboratory tests that included: glycemia, cholesterol (LDL, HDL), triglycerides and blood count full. The blood pressure was taken before starting periodontal therapy and the second blood pressure record was taken at the reevaluation appointment. Quantification of peripheral blood cytokines was performed using the Milliplex Inflammation Human Cytokine kit.
Results: All cytokine levels measured in the sample patients decreased from the initial examination to reassessment. The IL-1 had an initial value of 0.53 pg / ml, the IL-4 of 2.98 pg / ml, IL-6 of 0.31 pg / ml, IL-8 of 9.17 pg / ml, the IL-10 of 1.15 pg / ml, IL-12P70 of 1.82 pg / ml, IL-17A of 3.8 pg / ml, TNF of 15.84 pg / ml and VEGF of 152.10 pg / ml. The measures after periodontal treatment were: IL-1 of 0.30 pg / ml, IL-4 of 1.23 pg / ml, IL-6 of 0.24 pg / ml, IL-8 of 7.79 pg / ml, IL-10 of 0.86 pg / ml, 0.94 pg / ml IL-12P70, 3.19 pg / ml IL-17A, 14.29 pg / ml TNF and 101.55 pg / ml VEGF. The cytokines that reflected a statistically significant difference were IL-1B and VEGF (p = 0.04 and p = 0.004).
Conclusions: The hypertensive patients with periodontitis undergoing non-surgical periodontal treatment presented a decrease in the levels of pro-inflammatory cytokines evaluated, although the only ones that presented a statistically significant difference were VEGF and IL-1B
