Uncertainty in the patient with acute leukemia at the beginning of treatment : a care proposalResumo
Objetivo: Determinar el cuidado de Enfermería dirigido al paciente con Leucemia Aguda (LA) que asiste a tres unidades de hematología de tres instituciones de 4to nivel, basado en el nivel de incertidumbre que se genera al diagnóstico e inicio del tratamiento. Diseño: Estudio cuantitativo, descriptivo, transversal realizado con 21 pacientes adultos en la ciudad de Bogotá. Se utilizó el instrumento (MUIS- ADULT) Escala de incertidumbre ante la enfermedad Mishel. Los datos fueron procesados en R Software para el análisis de correspondencia múltiple. Para el análisis bivariado se aplicó la prueba Chi Cuadrado y Kruskal Wallis. Resultados: El nivel de incertidumbre fue alto en el 52,4% y medio en el 42,9% de la muestra. por dimensiones, se evidenció que la más afectada fue Capacidad Cognitiva 60,4%. El análisis de correspondencias múltiples no evidenció relación entre las variables del instrumento y el tipo de enfermedad, edad o sexo. No se evidenció diferencia en las respuestas dependiendo el tipo de diagnóstico médico. Se realizó un análisis bivariado asociando la escolaridad y el nivel de incertidumbre con una significancia de 0,10. Conclusiones: Los pacientes diagnosticados con LA presentan un grado de incertidumbre regular y alto. En respuesta se plantea la propuesta de Cuidado al paciente basado en los factores que aumentan la incertidumbre. No existe evidencia que describa el fenómeno de la Incertidumbre en el paciente adulto con LA, lo que plantea una oportunidad en la investigación de aportar al conocimiento de enfermería y conducir al mejoramiento de las prácticas de cuidado.
Abstract
Objective: To determine the Nursing care aimed at the patient with Acute Leukemia (LA) who attends three hematology units of three 4th level institutions, based on the level of uncertainty that is generated at the diagnosis and onset of treatment. Design: Quantitative, descriptive, cross-sectional study conducted with 21 adult patients in the city of Bogotá. It is specifically the instrument (MUIS-ADULT) Uncertainty scale before Mishel disease. The data was processed in R Software for multiple correspondence analysis. For the bivariate analysis, the Chi Square and Kruskal Wallis test was applied. Results: The level of uncertainty was high in 52.4% and average in 42.9% of the sample. By dimensions, it is evident that the most limited was Cognitive Capacity 60.4%. The analysis of multiple correspondences without evidence relationship between the variables of the instrument and the type of disease, age or sex. There is no evidence difference in the answers identified the type of medical diagnostic. A bivariate analysis was performed associating schooling and the level of uncertainty with a significance of 0.10. Conclusions: Patients diagnosed with LA have a regular and high degree of uncertainty. In response, the proposal of Patient Care is proposed based on the factors that increase uncertainty. There is no evidence describing the phenomenon of Uncertainty in the adult patient with LA, which poses an opportunity in research to contribute to nursing knowledge and lead to the improvement of care practices.
