Experiences of pregnancy, childbirth and the imaginary of mothers over their premature infants belonging to the Kangaroo Mother Care Program, residents in BogotáZusammenfassung
La maternidad es vista como un proceso en el cual, previamente al embarazo, hay expectativas individuales y sociales frente al rol materno que involucra las interacciones entre el bebé, la madre, y la familia. Estos ideales se consolidan durante el embarazo, sin embargo, el parto de un prematuro representa una contradicción frente a estas expectativas, pues las condiciones estresantes a las que la diada es sometida no son contempladas bajo un marco referencial tradicional, lo que podría dificultar la apropiación del rol materno y por ende las habilidades de cuidado frente al neonato.
El presente proyecto es una sistematización de la experiencia y aprendizajes adquiridos en la práctica familias, vínculos y violencias, cuyo objetivo es caracterizar la experiencia del embarazo y parto prematuro e identificar las variables que influyen en el imaginario y la calidad de cuidado de la madre sobre su bebé.
Para ello se realizó una entrevista semi estructurada por medio de la cual se identificaron las experiencias maternas durante el parto y embarazo. Posteriormente, se indagóel imaginario materno a través de la prueba proyectiva Machover(1949) adaptada para las madres de prematuros. Se obtuvo una muestra de 32 madres cuya edad oscila entre los 19 y 39 años, todas pertenecientes al Programa Madre Canguro Integral -PMCI- ubicado en el hospital San Ignacio.
Los resultados dieron cuenta de que las emociones prevalentes durante la gestación tales como felicidad, sorpresa, angustia y tristeza se clasifican de acuerdo con el tipo de embarazo -deseado, no deseado e inesperado-. Además, el apoyo de la familia y/o pareja fue fundamental para la aceptación del embarazo. Por otro lado, y con respecto al parto, la percepción positiva de éste se relacionó con la comunicación asertiva por parte del personal médico hacia la madre, haber experimentado poco dolor en el procedimiento y al momento del alumbramiento haber tenido contacto piel a piel con el bebé.
En lo que concierne a la imagen mental, todas las madres estaban consolidando la imagen real del neonato, pues la experiencia frente a la prematurez implicó una resignificación ante las expectativas de un bebé a término. Se concluye que la caracterización de la experiencia del embarazo, parto y la imagen mental brindan una mirada integral frente a la comprensión de la maternidad prematura. Adicionalmente, se cree que la prueba de Machover adaptada junto con la entrevista semi estructurada son herramientas que dan cuenta de la adherencia materna ante la posición canguro, la lactancia exclusiva y la estimulación del bebé, esbozando pautas de crianza maternas representando así un tamizaje de factores de riesgo y factores protectores.
Abstrakt
Motherhood is seen as a process in which, prior to pregnancy, there are individual and social expectations regarding the maternal role that involves the interactions between the infant, the mother, and the family. These ideals are consolidated during pregnancy, however, the delivery of a premature infant represents a contradiction against these expectations, since the stressful conditions are not contemplated under a traditional frame of reference, which could hinder the appropriation of the maternal role and therefore the care skills against the newborn.
The present project is a systematization of the experience and learning acquired in the practice of families, links and violence, whose objective is to characterize the experience of pregnancy and premature birth and to identify the variables that influence the imaginary and the quality of care of the mother about her baby.
For this, is used a semi-structured interview which identified maternal perceptions and emotions during childbirth and pregnancy. Subsequently, the maternal mental image is investigated through the Machover projective test (1949) adapted for mothers of premature babies. The sample obtained was 32 subjects whose age ranges were between 19 and 39 years, all belonging to the Integral Mother Kangaroo Program -IMKP- located in San Ignacio hospital.
The results showed that the prevalent emotions during pregnancy such as happiness, surprise, anguish, and sadness were classified according to the type of pregnancy - desired, unwanted, and unexpected. Furthermore, the support of the family and / or partner was essential for the acceptance of the pregnancy. On the other hand, and regarding childbirth, the positive perception of this was related to assertive communication by the medical staff towards the mother, have experienced little pain in the procedure and at the time of delivery have had skin-to-skin contact with the baby.
As to the mental image, all the mothers were consolidating the real image of the newborn, since the experience with prematurity implied a resignification before the expectations of a full-term baby. It is concluded that the characterization of the experience of pregnancy, childbirth and the mental image provide a comprehensive view of the understanding of premature motherhood. Additionally, it is believed that the adapted Machover test and the semi-structured interview are tools that account for maternal adherence to the kangaroo position, exclusive breastfeeding and stimulation of the baby, outlining patterns of maternal upbringing, thus representing factor screening risk and protective factors.
