Microorganisms associated with periodontitis in subjects treated at the clinic of the Faculty of Dentistry of the Pontificia Universidad Javeriana for the formation of a model of polymicrobial biofilms in vitroResumen
La periodontitis es una enfermedad infecciosa inflamatoria crónica, frecuente a nivel mundial y nacional, causada por biopelículas polimicrobianas formadas principalmente por bacterias periodontopatógenas y hongos levaduriformes. Esta afección conlleva a severas consecuencias a nivel individual y su tratamiento se basa en terapia antibiótica frente un solo patógeno específicamente bacteriano o antibióticos de amplio espectro, además de un tratamiento mecánico, de tipo invasivo como el desbridamiento dental para la eliminación de estas biopelículas. Por esto, este trabajo será, a futuro, la base para la búsqueda y determinación de nuevas alternativas de tratamiento, que inhiban la formación de dichos consorcios organizados en biopelículas.
Establecido lo anterior, el objetivo del presente proyecto fue describir los microorganismos asociados a periodontitis en sujetos atendidos en la clínica de la Facultad de Odontología de la Pontificia Universidad Javeriana para la formación de un modelo de biopelícula polimicrobiana in vitro. Para desarrollarlo,
en primera instancia se realizó la elección de muestras pertenecientes al banco de trabajo del Centro de Investigaciones Odontológicas (CIO), para su posterior cultivo en distintas diluciones por siembra masiva en agar sangre, tras 6 días de incubación a 37°C y en atmósfera de anaerobiosis se realizó el conteo de Unidades Formadoras de Colonias y la identificación de colonias utilizando MALDI-TOF. Seguido a esto, para la formación de la biopelícula de 96 h se preparó una suspensión microbiana a partir del cultivo de las tres muestras de un sujeto con periodontitis, para la inoculación de con 200 ul de la solución sin diluir y en distintas diluciones (Desde 10-1 a 10-6), cada 24 h durante 4 días.
Finalmente, se realizó el ensayo de viabilidad celular con sal de tetrazolio MTT, incubándolo a 37°C durante 1,5 horas en atmósfera de anaerobiosis, finalizado este tiempo se solubilizaron los cristales de formazán con DMSO y se realizó la lectura por espectrofotometría a una longitud de onda de 490 nm, en un lector de Microplacas para Absorbancia ELx800.
Las UFC/mL contadas en las 3 muestras fueron analizadas por estadística descriptiva, teniendo en cuenta la media, desviación estándar, y coeficiente de variación. Acerca de los microorganismos identificados en los dos grupos de sujetos, corresponden en su mayoría a especies asociadas a los complejos amarillo y azul. Por su parte C. albicans no fue identificada en ninguna de las muestras de los sujetos. Los resultados de la viabilidad no muestran diferencias estadísticamente significativas, sin embargo, la biopelícula de 72 h mostró mayor viabilidad en cada una de las cantidades de inoculo. Por último, el conteo de UFC/mL de las réplicas se analizó como se describió anteriormente; y los microorganismos recuperados e identificados en las réplicas corresponden en su mayoría a especies asociadas a los complejos amarillo y azul. No fue posible identificar bacterias asociadas al complejo rojo.
Se concluye que, a partir de la identificación de microorganismos en muestras de saliva, placa y surco gingival, se logró la recuperación de bacterias asociadas a los complejos amarillo, azul, y naranja. No se logró la recuperación de C. albicans, aunque esta se identificó en un primocultivo, posiblemente el agar utilizado para su recuperación favoreció el crecimiento de las bacterias y no de la levadura. Por otra parte, los microorganismos asociados al complejo rojo no fueron recuperados, posiblemente por la sensibilidad del método la cual no permite la recuperación de estos microorganismos asociados al complejo rojo. Además, la patogénesis de la periodontitis no recae únicamente en estos microorganismos asociados al complejo rojo si no que es problemática polimicrobiana que requiere estudios más profundos para establecer el rol de este consorcio microbiano. En cuanto al modelo de biopelícula polimicrobiano establecido en este estudio para la formación de una biopelícula a partir de microrganismos salvajes, nos evidencia que aquellos microorganismos que mayor adaptabilidad presentaron en el desarrollo de la biopelícula son aquellos asociados al complejo amarillo. Por último, se requieren 72 h del cultivo polimicrobiano para la recuperación y adaptabilidad al microambiente de diversos microorganismos asociados a los complejos amarillos, naranja, y azul.
Abstract
Periodontitis is a chronic inflammatory infectious disease, common worldwide and nationally, caused by polymicrobial biofilms formed mainly by periodontopathogenic bacteria and yeast fungi. This condition leads to severe consequences at the individual level and its treatment is based on antibiotic therapy against a single specifically bacterial pathogen or broad-spectrum antibiotics, in addition to a mechanical, invasive treatment such as dental debridement for the elimination of these biofilms. For this reason, this work will be, in the future, the basis for the search and determination of new treatment alternatives, which inhibit the formation of these consortia organized in biofilms.
Having established the above, the objective of this project was to describe the microorganisms associated with periodontitis in subjects attended in the clinic of the Faculty of Dentistry of the Pontificia Universidad Javeriana for the formation of a polymicrobial biofilm model in vitro. To develop it, in the first instance, the choice of samples belonging to the work bench of the Dental Research Center (CIO) was made, for subsequent cultivation in different dilutions by massive sowing in blood agar, after 6 days of incubation at 37 ° C and in anaerobic atmosphere the count of Colony Forming Units and the identification of colonies using MALDI-TOF was carried out. Following this, for the formation of the biofilm of 96 h a microbial suspension was prepared from the culture of the three samples of a subject with periodontitis, for the inoculation of with 200 ul of the undiluted solution and in different dilutions (From 10-1 to 10-6), every 24 h for 4 days. Finally, the cell viability assay was performed with MTT tetrazolium salt, incubating it at 37 ° C for 1.5 hours in anaerobic atmosphere, after this time the formalin crystals were solubilized with DMSO and the spectrophotometry reading was performed at a wavelength of 490 nm, in a microplate reader for Absorbance ELx800.
The CFU/mL counted in the 3 samples were analyzed by descriptive statistics, considering the mean, standard deviation, and coefficient of variation. About the microorganisms identified in the two groups of subjects, they correspond mostly to species associated with the yellow and blue complexes. C. albicans was not identified in any of the subjects' samples. The viability results do not show statistically significant differences, however, the 72h biofilm showed greater viability in each of the amounts of inoculum. Finally, the CFU/mL count of the aftershocks was analyzed as described above; and the microorganisms recovered and identified in the replicas correspond mostly to species associated with the yellow and blue complexes. It was not possible to identify bacteria associated with the red complex.
It is concluded that, from the identification of microorganisms in samples of saliva, plaque and gingival sulcus, the recovery of bacteria associated with the yellow, blue, and orange complexes was achieved. Recovery of C. albicans was not achieved, although this was identified in a primoculture, possibly the agar used for its recovery favored the growth of bacteria and not yeast. On the other hand, the microorganisms associated with the red complex were not recovered, possibly due to the sensitivity of the method which does not allow the recovery of these microorganisms associated with the red complex. In addition, the pathogenesis of periodontitis does not fall solely on these microorganisms associated with the red complex but is problematic polymicrobial that requires more in-depth studies to establish the role of this microbial consortium. As for the polymicrobial biofilm model established in this study for the formation of a biofilm from wild microorganisms, it shows us that those microorganisms that presented greater adaptability in the development of the biofilm are those associated with the yellow complex. Finally, 72 h of the polymicrobial culture is required for the recovery and adaptability to the microenvironment of various microorganisms associated with the yellow, orange, and blue complexes.
