Study of inter and intrafraction changes in image-guided hypofractionated radiotherapy in prostate cancerResumen
En este estudio se analizó el impacto de los cambios interfracción e intrafracción en los volúmenes blanco y órganos en riesgo, los efectos sobre la distribución de dosis, y cambios en los parámetros de registros recomendados durante el tratamiento de radioterapia hipofraccionada guiada por imágenes para cáncer de próstata.
Se tomó una muestra de los pacientes que fueron tratados con el protocolo de hipofraccionamiento moderado implementado en el Centro de Control del Cáncer adaptado partir del estudio CHHip (Conventional or Hypofractionated High Dose Intensity Modulated Radiotherapy for Prostate Cancer) que establece un fraccionamiento de 60 Gy en 20 sesiones durante 4 semanas (3Gy/Fx). Los pacientes fueron tratados con haz de 6MV, en modalidad RapidArc y el procedimiento fue guiado por imágenes de tomografía de haz cónico (CBCT), e imágenes planares para el seguimiento de la posición de marcas fiduciales implantadas.
El estudio se realizó con 36 pacientes, este grupo fue elegido de 83 pacientes bajo unos criterios de selección: contar con mínimo 18/20 CBCT durante su tratamiento (90 %), tener imágenes durante y/o al finalizar la sesión de tratamiento y no presentar inconvenientes en el recálculo de dosis. Para ellos, se realizó el corregistro de las imágenes de TC de simulación y los CBCT de las sesiones de tratamiento, se hizo contorneo de las estructuras de interés en los CBCT, se calculó la distribución de dosis para todas las sesiones y se extrajeron los datos requeridos para el análisis de los cambios ínter e intrafracción.
Los parámetros de registro para el estudio dosimétrico de los volúmenes blanco y los órganos en riesgo fueron tomados de los objetivos planteados en el protocolo, evaluando valores de dosis establecidos para cada estructura como el D98 % (dosis al 98 % del volumen) y el cubrimiento de los volúmenes por la dosis prescrita. Adicionalmente, se analizaron, los desplazamientos del volumen blanco a partir de la observación de las marcas fiduciales y estructuras anatómicas vecinas y el cambio en los volúmenes de los órganos en riesgo evaluados (recto y vejiga).
Resultados: Los márgenes calculados, basados en la ecuación de Van Herk [59] para lograr el objetivo de que el 90 % de los pacientes reciba el 95 % de la dosis, sin uso de IGRT fueron: 4.89 mm en dirección AP, 6.40 en dirección SI y 2.37 en dirección LR; con IGRT diaria fueron 3.22 mm en dirección AP, 3.28 en dirección SI y 1.52 mm en dirección LR, es decir que los márgenes implementados en el protocolo son pequeños de acuerdo a los desplazamientos que se presentaron en los pacientes de la muestra. La diferencia entre las dosis planificadas y administradas para el CTV1 y CTV2, no fueron superiores al 5 %. Para el CTV3, los parámetros V60Gy y V63Gy si tuvieron una variación superior al 5 % entre la planeación y la administración. Se evidencio que la variación del volumen de los OAR afecta el cumplimiento de los objetivos dosimétricos.
Abstract
This study analyzed the impact of interfraction and intrafraction changes on target volumes and organs at risk, effects on dose distribution, and changes in recommended recording parameters during image-guided hypofractionated radiotherapy treatment for prostate cancer.
A sample of patients was taken who were treated with the moderate hypofractionation protocol implemented at the Cancer Control Center adapted from the CHHip study (Conventional or Hypofractionated High Dose Intensity Mo-dulated Radiotherapy for Prostate Cancer) which establishes a fractionation of 60 Gy in 20 sessions during 4 weeks (3Gy/Fx). The patients were treated with a 6MV beam, in RapidArc modality and the procedure was guided by cone beam tomography (CBCT) images and planar
images to follow up the position of the implanted fiducial marks.
In the study, 36 patients were included , this group was chosen from 83 patients under two selection criteria: to have a minimum of 18/20 CBCT during their treatment (90 %), to have images during and/or at the end of the treatment session and not to present any inconvenience in the dose recalculation. For these, the simulation CT images and the CBCT images of the treatment sessions were co-registered, the structures of interest in the CBCTs were contoured, the dose distribution was calculated for all the sessions and the data required for the analysis of the inter- and intra-fraction changes were extracted.
The registration parameters for the dosimetric study of the target volumes and organs at risk were taken from the objectives established in the protocol, evaluating the dose values established for each structure such as D98 % (dose at 98 % of the volume) and the coverage of the volumes by the prescribed dose. Additionally, the displacements of the target volume from the observation of the fiducial landmarks and neighboring anatomical structures and the change in the volumes of the organs at risk evaluated (rectum and bladder) were analyzed.
Results: The calculated margins, based on the Van Herk equation [59] to achieve the objective of 90 % of patients receiving 95 % of the dose, without the use of IGRT were: 4.89 mm in AP direction, 6.40 in SI direction and 2.37 in LR direction; with daily IGRT they were 3.22 mm in AP direction, 3.28 in SI direction and 1.52 mm in LR direction, i.e. the margins implemented in the protocol are small according to the displacements that occurred in the patients of the sample.The difference between the planned and administered doses for CTV1 and CTV2, were not higher than 5 %. For CTV3, the V60Gy and V63Gy parameters did have a variation of more than 5 % between planning and administration. It was shown that the variation in the volume of the OARs affects the achievement of the dosimetric objectives.
